To fill a new prescription with the Home Delivery Pharmacy:

1. Complete the New Enrollment Form, and our team will set up your home delivery account. This form requests all of the information we need before we can process your first order, and it can be submitted with or without a prescription. If you’d prefer to set up your home delivery account over the phone, call our Customer Service team at 800-759-3203.
2. Request that your prescriber submit an electronic prescription to Serve You Rx Home Delivery Pharmacy or via fax at 866-494-0364.

Transfer Your Medications
To transfer your prescriptions to our Home Delivery Pharmacy, you can complete our Prescription Transfer Form. Alternatively, you can call us at 800-759-3203 with the name and phone number of your current pharmacy and your prescription number(s).

Refilling your prescriptions with Serve You Rx Home Delivery Pharmacy is simple. You can mail us a completed order form (included with your last delivery) or choose one of these easy options:
